b'KNOWING OUR PATIENTS Aging Gracefully ProgramNearly 8 in 10 Americans prefer to stay at home as they age.* However, to age safely in place, healthcare support and home modifications are often necessary to address fall risks and other issues.As part of the Aging Gracefully Program, THN partners with Community Housing Solutions and Cone Health to provide safe living conditions as people advance in age. The program is based on The Johns Hopkins CAPABLE (Community Aging in PlaceAdvancing Better Living for Elders) Model, a proven health program to help older Americans remain at home. THN was 1 of only 6 in the nation selected to participate in the national pilot study.With support from occupational therapy, nursing and home improvement specialists, qualifying older adults create goals for living safely and independently at home. They receive in-home visits from occupational therapists and registered nurses and professional home accessibility and safety modifications tailored to their needs.In 2022, 25 patients age 65 and older with income constraints and activities of daily living (ADL) issues completed the program.
