b'SPECIALTY CARE Convenient ScreeningIn 2022, to close healthcare gaps and coordinate patient care, THN clinic quality coordinators conducted more than 61,000 chart reviews, identified more than 12,000 quality gaps and made more than 5,000 patient outreach calls. To support patient follow through and positive outcomes, the THN clinic quality coordinators team launched a diabetic retinal screening program in 2021. In addition, the team organizes mobile screenings at participating practices and provides at home test kits directly to patients. Diabetic Retinal Screening Program In 2022, THNs Diabetic Retinal Screening Program scheduled and conducted 120 free retinal screening events at 41 provider locations. In addition to being at higher risk for glaucoma, cataracts and other eye problems, people with Type 1 and Type 2 diabetes are at risk for developing diabetic retinopathy, the leading cause of blindness among working-age Americans. Using THNs Intelligent Retinal Imaging Systems (IRIS) specialized camera with portable Remidio Innovative Solutions technology, trained THN clinic quality coordinators conduct diabetic retinal screenings. Securely uploaded images, which have a 95% gradeability rating, are shared with an ophthalmologist who provides results to the patients primary care provider within three days of the event. Providers communicate results to their patients and coordinate further evaluation or treatment, if needed.16'
