b'THNS HISTORY The Right Care, Right Place & Right Time2010 - 2011Triad HealthCare Network (THN) launches with a pay per performance and population health strategy to bring cost-efficient, high-quality care to the Piedmont Triad.2012 - 2014 The Centers for Medicare and Medicaid Services (CMS) selects THN to participate in the Medicare Shared Savings Program as an Accountable Care Organization (ACO). THN evaluates and forges strategic partnerships to manage healthcare costs.2015 HealthTeam Advantage, a Cone Health-sponsored Medicare Advantage Plan, launches.2016 THN is selected to participate as a Next Generation ACO, which:Builds on experience from the Pioneer ACO Model and the Medicare Shared Savings Program.Supports ACOs experienced in coordinating care for patients and whose provider groups are ready to assume higher levels of financial risk and reward.Provides quality, cost-efficient and patient-focused care.2017 - 2018 THN assumes all risk-based administrative services, adds commercial risk-based reimbursement plans and receives the National Association of ACOs Innovation Award. THN becomes 1 of 6 organizations in the nation selected for a national Aging Gracefully in Place study.2019 THN earns the Health Technology Assessment (HTA) 4 Star Quality Award and introduces its Elite Primary Care Model for practice onboarding, education and support.2020 Ranked #2 in the nation for its NextGen ACO Quality Score, THN achieves its highest revenue to date and responds to the COVID-19 pandemic: 35,000 care kits are sent to high-risk patients. Health Team Advantage receives an overall plan rating of 5 stars, the highest rating a plan can achieve, for 2020 and 2021. 2021 - 2022 THN achieves #1 NextGen ACO Quality Score in the nation for 2021 in its final year of participation in this ACO model. Patient outreach, embedded care and community partnerships expand. 2023 THN transitions to the ACO REACH (Realizing Equity, Access, and Community Health) model to further address health disparities.6'
