The most advanced technology or nationally recognized standards can only reach so far when someone cannot get to an appointment, afford a prescription or reliably access nutritious food. Financial strain forces impossible choices, and these pressures shape lives every day in our communities. Across the Piedmont Triad region, these barriers determine who stays well — and who does not. Cone Health’s approach acknowledges that reality. We see health not as a series of clinical encounters but as a continuum influenced by trust, stability and access. This requires meeting people where they are — in schools, churches, community centers and neighborhoods — and understanding the pressures that shape their lives long before they walk into a clinic or hospital. It also means listening carefully because the people living closest to the challenges often understand them best. Across our five-county service area, Cone Health partners with cities, counties, schools, nonprofits and residents to identify where need is greatest and what interventions will make the most meaningful impact. Public health data helps clarify where gaps exist; community voices illuminate why they persist and what might close them. These disparities are not the product of chance. They reflect differences in opportunity, access and support that we can address. Together, these insights guide programs that make care easier to reach and easier to use. Three initiatives illustrate how your support translates into measurable change: Expanding mobile medicine Mobile units bring primary and preventive care directly into neighborhoods where people otherwise go without it. These visits help residents manage chronic conditions, access screenings and receive timely care before issues escalate. Supporting maternal-fetal health In Guilford County, Black infants die at nearly four times the rate of white infants — a heartbreaking indicator of how urgently we must improve maternal health. Your investment expands access to care in the first trimester and ensures mothers have the resources and stability they need for safe pregnancies and healthy babies. Developing the Chavis Health Hub Cone Health will create a neighborhood-based center that brings primary care, behavioral health, prevention and social-support services together in one accessible destination. Guided by Guilford County’s Community Health Assessment, the hub removes barriers tied to transportation and fragmented care, reduces preventable emergency-department use and delivers measurable improvements for families today while building healthier neighborhoods for generations.
