Care Without Delay (CWD) is a system-wide initiative
designed to help patients get admitted faster, receive care
more efficiently and head home sooner.
Alamance Regional Medical Center was the first Cone Health
hospital to initiate CWD work streams. In July of 2025, a
steering team identified 13 work streams as opportunities.
One focus of the Alamance Regional CWD team is patients
who are medically ready for discharge and do not have a safe
discharge plan. A multi-disciplinary team meets weekly to
review and implement action plans to get patients to a safe
discharge location.
Successful outcomes include:
• A 75% reduction in the daily census of patients who
were medically ready for discharge but did not have a
    safe discharge plan.
• A new Epic Care Management dashboard was created for
    this patient population.
• The length of stay has decreased.
• Mobility for patients has increased.
• The work has increased visibility of many key
    performance indicators.
In 2026, Moses Cone Hospital, Wesley Long Hospital
and Annie Penn Hospital also started CWD teams. CWD
improvements mean patients spend less time waiting,
experience smoother transitions and receive care in the most
appropriate setting.

The Cone Health
Mobile Health
Program continues to
transform how care is
delivered by meeting
patients in their own
communities. What
began as a single
mobile effort has expanded into five mobile health units, and
the Mammography Screening Unit, which is a partnership
with DRI.
Mobile units visit neighborhoods, schools, churches and
community organizations in Guilford, Rockingham, Alamance,
Randolph and Forsyth counties.
The program includes the following units:
• Mobile Medicine: primary care
• Mobile Screening: screenings, vaccines and education
• Healthy Heart: screenings, education and follow-up
connections to care for high blood pressure, diabetes,
    high cholesterol and obesity
• OB/GYN: obstetrical and gynecologic care, education
    and referrals
• Mobile Mammography
• Mobile Cancer Screening: screenings for breast, cervical,
prostate, colorectal, lung and oral cancers.
The program’s impact extends far beyond clinical services.
Mobile health teams build trust within the community, identify
unmet needs in real time and connect patients to ongoing
care and resources.
