Nurse Sensitive Indicators
Cone Health benchmarks Nurse Sensitive Quality Indicators
against national benchmarks. Following is a summary of how
Cone Health is performing in three categories.
Falls With Injury
Patient falls resulting in injury remain one of the most
important measures of patient safety. Falls during
hospitalization can lead to longer recoveries, reduced mobility
and increased complications. Preventing falls is a critical
focus for nursing teams. Falls with injury is an area targeted
for improvement. During 2024 and 2025, two Cone Health
campuses outperformed the national benchmark in 4 out of 8
quarters and 5 out of 8 quarters.
Pressure Injuries
A hospital-acquired pressure injury occurs when a patient
develops skin breakdown during hospitalization that was not
present when they arrived. These injuries can lead to pain,
infection, longer hospital stays and increased health care costs.
Because many pressure injuries are preventable, nurses play
a critical role in early assessment, prevention and treatment
through vigilant monitoring, repositioning, skin care and
collaboration with the care team. Cone Health is outperforming
national benchmarks, with four campuses scoring above
national measures in five of the five quarters measured.
Tobacco and Substance Screening
CMS screening measures assess whether patients are
routinely screened for tobacco and substance use and
offered appropriate counseling, treatment or referral
services when needed. By identifying risks early and
connecting patients with supportive resources, teams
help improve health outcomes, prevent complications
and promote overall wellness. This measure is reported
in our in-patient behavioral health areas. Cone Health is
performing well against national benchmarks.
Driving Quality
Cone Health nurses play a vital role in safeguarding
patient care through their keen observation and
commitment to quality. The Good Catch Award recognizes
team members who identify and report near misses,
creating opportunities to improve processes and systems
before potential safety events reach patients. Over the last
two years, 519 Good Catch Awards have been presented
in recognition of nurses’ vigilance, engagement and
dedication to advancing a safer clinical environment.
Emergency Department Metrics
Emergency Department nurses are essential to
coordinating patient care, ensuring safe, timely and
efficient delivery of services in a dynamic clinical setting.
During FY 2025, the Emergency Department achieved
a 35% reduction in patient falls with injury compared
with FY 2024. This improvement reflects the nursing
team’s sustained commitment to patient safety through
consistent fall risk assessments, timely interventions,
enhanced staff awareness and adherence to evidence-
based fall prevention practices. This achievement also
reinforced patient and family confidence in the quality and
safety of care provided within the Emergency Department.
